PROCEDURES USED FOR THE SECOND TRIMESTER ABORTION - DILATION AND EVACUATION, ALSO CALLED THE D & E OR LAMINARIA EVACUATION This procedure, which is done under local or general anesthesia, is divided into two parts. First, the practitioner will insert a thin rod made of either sterilized seaweed (laminaria) or an extremely thin plastic strip into the cervix one or two days before the scheduled abortion. Over that time, the rod will absorb moisture and gradually dilate the cervix. This can cause cramping, which can be relieved by over-the-counter painkillers, but can be quite uncomfortable for women who have not had biological children. If the procedure is being done after 18 weeks and up to 24 weeks, the dilation process will happen over one or two days, with additional dilateria put into the cervix to open it further, after which the fetus will be removed using medical instruments. Dr. E. Hakim-Elahi, medical director for Planned Parenthood of New York City, urges women who are having this later procedure done to go to a hospital where bleeding and other complications can be managed on the spot. However, most 2nd trimester abortions are done in free-standing clinics. At the later stage, because the fetus is more developed, your practitioner will have to make sure that all fetal structures have been removed. Complications result more often than with early abortions. Many practitioners feel that in the right hands, the D & E procedure is preferable to the amnio infusion procedure described next. The trouble is, it can be hard to find a practitioner trained in D & E in parts of the country—and practitioner skill is very important for this procedure. Sometimes, clinics that do early abortions can give references to skilled late-abortion practitioners in your area. You may have to travel to find someone reputable and experienced.
