The Grady report only offers an estimate for estrogen replacement alone (ERT), since we have more data on ERT than on HRT. If you tally up the reduction in risk of heart disease and osteoporosis, and place it against the increased risk of endometrial cancer and breast cancer, you end up with an increased life expectancy for the average 50-year-old white woman of just under one year. In other words, the benefits outweigh the risks as far as life span is concerned. What that means to you is another story altogether. If you have a very high risk of heart disease (let's say you have high cholesterol, high blood pressure, you're overweight, or have a strong family history of heart disease) and you're at low risk for breast cancer, hormone replacement might add years to your life. If, on the other hand, you're at high risk for breast cancer, and very low risk for heart disease and bone loss, the scale for you might tip the other way—against taking hormones. This is all the more reason to have a complete health assessment at the time of menopause: if you find out how your heart and bone health are holding up, take a complete family history of disease, and have your breasts examined with mammography, you'll get a better idea of where you stand. As a generalization, Dr. Grady says that hormone therapy is probably best for women who have already had hysterectomies, and for women who already have heart disease or who have many risk factors for heart disease.
