ENDOMETRIOSIS: TREATING ENDOMETRIOSIS (RELIEVING PAIN AND ELIMINATING LESIONS)

There is no cure for endometriosis, but treatments can help a great deal in relieving pain and eliminating lesions. Surgery is usually the first step—to remove as many lesions and adhesions as possible and eliminate pain at least for now. As mentioned above, it's the best way to restore fertility to women with endometriosis, although not guaranteed.

Often, though, lesions will recur after surgery. That's where hormonal therapy comes in. There are several drug choices, all of which affect your hormones in some way. Danazol, which is FDA-approved for the treatment of endometriosis, keeps you from ovulating by preventing the surge of luteinizing hormone and inhibiting the enzyme receptors in the endometrial lesions. The drug works pretty well, but has significant unpleasant side effects, primarily causing masculinization: for example, hair growth on the face, a lowered voice, weight gain and increased muscle mass. Some women will not risk enduring these side effects. Another group of drugs used for endometriosis include progestins, the most common of which in the U.S. is Provera. These are not approved by the FDA for endometriosis, but Dr. Olive finds that they help some women and have much milder side effects and cost less than some other therapies. Recently approved by the FDA for endometriosis treatment are the GnRH agonists, a group of relatively costly drugs like Lupron, Synarel and others that produce a medical menopause by cutting out estrogen in the body. The side effects are the same as during menopause, when estrogen is depleted from the body: everything from hot flashes to bone loss. These side effects are thought to be reversible, however, when the drugs are stopped (they should only be taken for six months). All of the above-mentioned drugs reduce both pain and the number and size of endometrial lesions.
