WEIGHING RISKS AND BENEFITS: A LOOK AT THE STATISTICS - DOWN SIDE OF HORMONE THERAPY We've been talking about the benefits of hormone therapy. Now let's take a look at the down side. Estrogen alone increases your risk of endometrial cancer (cancer of the lining of the uterus). The average 50-year-old Caucasian woman has only a 2.6% chance of getting endometrial cancer; if she takes estrogen replacement alone, her risk jumps to 19.7%—a very significant increase. If you no longer have a uterus (you've had a hysterectomy) this is of no concern to you. If you do still have your uterus, however, experts recommend adding the hormone progesterone to your treatment regimen. When you take both estrogen and progesterone together (either continuously, or in a cyclic fashion we'll discuss later) you erase that increased risk of endometrial cancer. It's easy to understand why: progesterone promotes the shedding of the uterine lining, just as it used to shed with your menstrual period. So instead of building up and potentially becoming cancerous, the lining is sloughed off regularly. The trouble is that progesterone causes many unpleasant side effects for some women, as we'll discuss below. If you still have your uterus, can't tolerate progesterone, and want to take estrogen alone, you'll be advised to have an endometrial biopsy (a test of the lining of your uterus) every year, or whenever abnormal bleeding occurs. *416/162/1* General health |
